We are currently accepting new patients!

If you are interested in becoming a client and establishing your pet with us, please follow the steps below:

Step 1: Please complete our 3 new client forms located  below which include the: Pet Information Form, Guardian Information Form and Client Acceptance Form. (If you are interested in setting up an appointment for more than one pet, we ask that you complete a “Pet Information Form” for each of them)

Step 2: Once your forms are completed, please email your forms back to us at holisticpetvet@holisticpetvetclinic.com.

Step 3: Once your forms are received, our staff will add your information into our system and will give you a call within 1-2 business days to schedule your initial appointment and collect a one-time $100 deposit.
